• Child health (immunisations, child health surveillance)

• Family planning and contraceptive advice, IUD and IUCD (by appointment). If you have been advised by the surgery to submit a contraceptive pill review, please complete the Contraceptive Pill Review form
• Men’s health (sexual dysfunction, prostate and urinary tract problems)
• Chronic disease management (diabetes, high blood pressure, high cholesterol, COPD, asthma etc.)
  • If you have been advised by the surgery to submit an annual review of your asthma symptoms, please complete the Asthma Review form
  • If you have been advised by the surgery to submit your blood pressure readings on a regular basis, please complete the Blood Pressure Review form
  • If you have been advised by the surgery to submit a COPD assessment, please complete the COPD Assessment form
• Smoking cessation – If you have been advised by the surgery to submit a smoking review on a regular basis, please complete the Smoking Review form
• Wound care and dressings
• Minor illnesses
• Travel health and vaccinations – If you are travelling abroad please make sure you complete the Travel Risk Assessment form in plenty of time to arrange any necessary vaccinations
• Healthy living (exercise advice and prescription, dietary advice, health promotion)
• Antenatal and maternity care
• Mental health (depression, anxiety, stress)
• Patients with chronic diseases will be offered a yearly comprehensive medical check
